腱板断裂症例に対して理学療法士・作業療法士が関わる機会は多いと思います．
保存療法の方はもちろんですがARCRに代表されるような術後の症例を担当する機会も増えております．
腱板機能の中でも重要なのが棘上筋の機能です．
棘上筋の機能が低下した場合にはさまざまな代償的な戦略を用いて上肢を挙上することになりますが，どういった戦略が用いられるのでしょうか？
今回は肩腱板断裂症例における棘上筋機能の代償的戦略を考えうえで参考になる論文をご紹介させていただきます．

研究の背景
Rotator cuff tear is a common shoulder injury that causes shoulder dysfunction and pain. Although surgical repair is the primary treatment for rotator cuff tear, it is well recognized that impaired force exertion of muscles connecting to the involved tendon and subsequent complemental change in the force exertion of synergist muscles persist even after repair. This study aimed to identify the compensation strategy of shoulder abductors by examining how synergist muscles respond to supraspinatus muscle force deficit in patients with rotator cuff repair.
腱板断裂は肩の機能障害や痛みの原因となる一般的な肩の傷害であります．
腱板断裂は外科的修復が主な治療法でありますが，関与する腱に接続する筋の力発揮の障害とそれに続く共同筋の力発揮の代償的変化が修復後も持続することがよく知られております．
この研究では腱板修復後の症例を対象として棘上筋の筋力低下に対する共同筋の反応を調べることにより，肩外転筋の代償戦略を明らかにすることを目的としております．
研究の方法
Muscle shear modulus, an index of muscle force, was assessed for supraspinatus, infraspinatus, upper trapezius, and middle deltoid muscles in repaired and contralateral control shoulders of 15 patients with unilateral tendon repair of the supraspinatus muscle using ultrasound shear wave elastography while the patients passively or actively held their arm in shoulder abduction.
棘上筋の片側腱を修復した15例の症例の修復肩と対側の対照肩において，症例が腕を受動的または能動的に肩外転させた状態で超音波せん断エラストグラフィーを用いて，筋力の指標である筋せん断弾性率を評価しております．
研究の結果
In the repaired shoulder, the shear modulus of the supraspinatus muscle declined, whereas that of other synergist muscles did not differ relative to that of the control. To find the association between the affected supraspinatus and each of the synergist muscles, a regression analysis was used to assess the shear moduli at the population level. However, no association was observed between them. At the individual level, there was a tendency of variation among patients with regard to a specific muscle whose shear modulus complementarily increased.
修復された肩では棘上筋のせん断弾性率は低下しましたが，他の共同筋のせん断弾性率は対照のそれと比べて差がありませんでした．
罹患した棘上筋と各共同筋との関連を見つけるために，回帰分析を用いて集団レベルでの剪断弾性率を評価しております．
しかしながら両者の間に関連性は認められませんでした．
個人レベルでは代償的にせん断弾性率が上昇する特定の筋について，症例間でばらつきがある傾向がみられました．
研究の結論
These results suggest that the compensation strategy for supraspinatus muscle force deficit varies among individuals, being non-stereotypical in patients with rotator cuff injury.
これらの結果から棘上筋機能低下に対する代償戦略は個人差があり，腱板損傷症例における代償的戦略は非定型的であることが示唆されました．
今回は肩腱板断裂症例における棘上筋機能の代償的戦略を考えうえで参考になる論文をご紹介させていただきました．
今回の結果から考えると腱板断裂症例における棘上筋の機能低下に対する代償的戦略は個体間でもかなりバラツキがあるということですね．
どういった代償戦略を用いているか，その代償戦略が継続されることによる様々なリスクを考慮した上で代償的な戦略から正常な動作パターンへの修正を図ることが求められますね．
