大腿骨近位部骨折後には股関節外転筋力の低下が問題となることが多いです．

股関節外転筋群の中でも中殿筋の果たす役割が重要であると考えられてきましたが実際のところ中殿筋の果たす役割というのはどうなのでしょうか？

今回は大腿骨近位部骨折後の中殿筋の役割を考えるうえで参考になる論文をご紹介させていただきます．

研究の目的

We investigated the association between the cross-sectional area (CSA) of the gluteus medius muscle (GMM) and activities of daily living in patients with hip fractures.

この研究では大腿骨近位部骨折例における中殿筋断面積（CSA）と日常生活動作との関連を検討することを目的としております．

研究の方法

This retrospective cohort study comprised 111 patients aged ≥65 years who underwent hip fracture rehabilitation. The CSA of the GMM was measured using computed tomography scans in the early stages of hospitalization.

この後方視的コホート研究では大腿骨近位部骨折に対してリハビリテーションを施行した65歳以上の111例を対象としております．

入院初期にCTスキャンを用いて中殿筋の筋断面積を測定しております．

研究の結果

The group with decreased CSA of the GMM had a median GMI ≤17 cm2/m2 for male patients and ≤16 cm2/m2 for female patients. Patients in the group with decreased CSA of the GMM had lower functional independence measure gains than those in the control group. After adjusting for confounders, we found that decreased CSA of the GMM was significantly associated with lower functional independence measure gains (β: -0.432, p < .001).

中殿筋の筋断面積が低下していたグループのGMI中央値は男性で≦17cm2/m2，女性で≦16cm2/m2でありました．

中殿筋の筋断面積が減少した群では，対照群よりも機能的自立度測定値の上昇が低い結果でありました．

交絡因子を調整した結果，中殿筋の筋断面積の減少は，機能的自立度測定値の低下と有意に関連しておりました（β：-0.432，p＜0.001）．
