TKA後の滑膜炎には注意が必要

TKA術後早期って夜間痛や安静時痛を訴えられる方が多いです．

夜間痛や安静時痛の原因もさまざまですが，原因の1つとして滑膜炎が考えられます．

しかしながらTKA後の滑膜炎についてはその詳細は不明な点が多いのも実際です．

今回はTKA後の滑膜炎には注意が必要であることを示唆する研究論文をご紹介させていただきます．

研究の目的

In this study, we examined the incidence and extent of synovitis and its association with pain in patients in the subacute phase after total knee arthroplasty (TKA).

この研究では人工膝関節全置換術（TKA）後の亜急性期にある症例を対象として，滑膜炎の発生率や程度，痛みとの関連について検討することを目的としております．

 

 

 

 

 

 

 

研究の方法

The study included 50 patients who underwent unilateral TKA, and assessments were conducted one month postoperatively at four sites: the medial and lateral sides of the patella, the upper part of the patella, and the surgical side patellar fossa. The intensity of resting pain, the pressure pain threshold (PPT), and the degree of synovitis were evaluated using Power Doppler ultrasonography.

片側人工膝関節全置換術を受けた患者50例を対象として，術後1ヶ月に膝蓋骨の内側と外側，膝蓋骨上部，手術側の膝蓋骨窩の4部位で評価を行っております．

安静時痛，圧痛閾値（PPT），滑膜炎の程度は，パワードップラー超音波検査を用いて評価しております．

 

 

 

 

 

 

 

研究の結果

The results showed synovitis in 40-60% of patients one month after TKA. There was a weak to moderately significant positive correlation between the degree of synovitis and the intensity of resting pain, particularly in the medial and lateral side of the patella and patellar fossa regions.

人工膝関節全置換術施行1ヵ月後には40～60％の症例に滑膜炎が認められました．

滑膜炎の程度と安静時痛の強さとの間には，弱～中等度の有意な正の相関が認められ，特に膝蓋骨の内側と外側，膝蓋骨窩領域に顕著でありました．

 

 

 

 

 

 

 

研究の結論

Synovitis in the subacute phase after TKA may become a target for treatment, and objectively assessing the presence and degree of synovitis using ultrasound Power Doppler could contribute to effective physical therapy.

人工膝関節全置換術後の亜急性期における滑膜炎は，治療のターゲットになる可能性があり，超音波Power Dopplerを用いて滑膜炎の存在と程度を客観的に評価することは，効果的な理学療法に貢献する可能性があります．

 

今回はTKA後の滑膜炎には注意が必要であることを示唆する研究論文をご紹介させていただきました．

今回の結果から考えるとかなり効率に滑膜炎を合併されている方が多いということですね．

また滑膜炎と安静時痛に有意な関連があったという点もポイントですね．

膝蓋骨周囲の安静時痛がある場合には滑膜炎を疑ってみる必要がありますね．
