研究の背景
Background: Gluteal tendinopathy (GT) is found in 20 to 25% of patients undergoing total hip arthroplasty (THA). Despite this, there is a scarcity of literature assessing the association between GT and THA outcomes. The aim of this study was to evaluate whether intra-operative diagnosis of GT negatively affected postoperative outcomes.

しかしながら殿筋群の腱障害とTHA後のアウトカムとの関連を評価した文献は少ないのが現状です．
この研究では殿筋群の腱障害が術後アウトカムに悪影響を及ぼすかどうかを評価することを目的としております．
研究の方法
Methods: Consecutive patients undergoing primary THA for osteoarthritis via a posterior approach over 5 years were recruited in a prospective study. Gluteal tendinopathy (GT) was assessed and graded at the time of surgery, but not repaired. A total of 1,538 (93%) completed the PROMs at 1 year after surgery and were included in the analysis. The Patient Reported Outcome Measures (PROMs) included the Oxford Hip Score (OHS), Hip Disability and Osteoarthritis Outcome Score Joint Replacement (HOOS JR) and EuroQol 5-Dimension (EQ-5D), and were collected preoperatively and one year after THA.
後方アプローチによる変形性関節症に対するprimary THAを5年以上受け ている連続症例を前向き研究でリクルートしております．
殿筋群の腱障害は手術時に評価しグレード付けを行っておりますが，修復は行われておりません．
合計1,538例（93％）が術後1年目にPROMを記入し，解析に組み入れられております．
患者報告アウトカム指標（PROM）にはOxford Hip Score（OHS），Hip Disability and Osteoarthritis Outcome Score Joint Replacement（HOOS JR），EuroQol 5-Dimension（EQ-5D） が含まれ，THA術前と術後1年目に収集しております．
研究の結果
Results: The gluteal tendons were graded as 4 distinct grades: normal (n = 1,023, 66%), tendinopathy but no tear (n=337, 22%), partial thickness tear (n=131, 9%), and full thickness tear (n=47, 3%). The occurrence of GT was associated with age, body mass index (BMI), and sex. There was no significant difference in baseline OHS or HOOS JR scores according to GT grade. As GT grade increased, lower median 1-year OHS (P = 0.001) and HOOS JR (P = 0.016) were observed. This association was confirmed by linear regression analysis with 1-year OHS (B = 0.5, 95% CI = -0.9 to -0.1, P = 0.011) when controlled for age and sex.
殿筋腱は正常（n=1,023、66％），腱障害を認めるも断裂なし（n=337、22％），部分断裂（n=131、9％），完全断裂（n=47、3％）の4段階に分類しております．
殿筋群の腱障害の発生は年齢， BMI，性別と関連しておりました．
ベースラインのOHSおよびHOOS JRスコアには殿筋群の腱障害グレードによる有意差はありませんでした．
殿筋群の腱障害の重症度が高くなるにつれて，1年間のOHSの中央値（P = 0.001）およびHOOS JRの中央値（P = 0.016）が低くなりました．
この関連は年齢と性別で調整した場合の1年OHS（B = 0.5、95％CI = -0.9～-0.1、P = 0.011）との線形回帰分析でも確認されました．
研究の結論
Conclusion: Gluteal tendinopathy was commonly observed and was associated with inferior 1-year PROMS in patients undergoing THA via posterior approach. Increasing degree of tendinopathy was a negative prognostic factor for outcomes and patient satisfaction.
殿筋群の腱障害は後方アプローチでTHAを受けた症例によくみられ，1年PROMSの低下と関連しておりました．
殿筋群の腱障害の重症度が高くなることは，アウトカムと患者満足度にとって予後不良因子でありました．
今回は殿筋群の腱障害がTHA後のアウトカムと関連するのかどうかを考えるうえで参考になる論文をご紹介させていただきました．
後方アプローチによるTHAの場合には殿筋群の腱障害をどこか頭の隅にでも置いておく必要があることが示唆される結果ですね．
